Change of Surname in the Commercial Register

I changed my surname after getting married; how can I record this change in the commercial register?
ANSWER:
For a change of surname in the commercial register, it is necessary to submit a form – an application for a change entry. The form is available on the website of the Ministry of Justice of the Slovak Republic, and you need to choose it according to the form of the business company. If it concerns the change of surname of a managing director, the box “managing director / statutory body” is checked, and all new details of the managing director are filled in the left column, meaning not only the surname but also all columns related to the managing director – first name, surname, address, date of birth, personal identification number, and the date of assuming office (this date is in the extract from the commercial register). In the right column for deletion, all details of the managing director are again entered, but with the old surname. The only difference between the left and right columns will be the surname; the other details will be the same in both columns.
If the managing director is also a partner of the company, the box “partner” is checked, and again all details with the current surname are filled in the left column, and in the right column, the details proposed for deletion, i.e., the old surname. Again, the left and right columns will be the same except for the surname.
This procedure applies also to other changes concerning the same managing director, for example, a change of the managing director’s address.
The application must be accompanied by a document proving the change. This document can be a sworn statement of the change of data with a notarized signature, or it is possible to submit a copy of the new identity card with the new data. In the sworn statement, it is sufficient to state that the person changed their surname from the original surname “xy” to the new surname “yx.”
