Attachments to the Application for Slovak Citizenship
QUESTION
Hello, I would like to submit an application for granting citizenship. What form should the employer’s confirmation have? Does the confirmation from the insurance company stating that I am insured need to be the original with a stamp, or can I download it online? If I am also studying, do I need to provide a confirmation of my studies as well?
ANSWER:
- The mentioned confirmation does not have a prescribed form, and we do not have a template for it. It must include, in particular, the following: confirmation of income tax payment and payment of contributions for public health insurance, social insurance, and pension savings.
- Yes, it must be the original with a stamp, and the insurance company branch should be able to issue it to you on the spot. However, we have encountered cases where it was also necessary to provide the European Health Insurance Card, if the applicant possessed one.
- If you are studying, we recommend submitting a confirmation of your studies. This document should be issued by the relevant school. Please note that even if you are a student, you must also provide a confirmation of tax and fee payments from the municipality where you live. This document serves as proof that the applicant has no outstanding debts to the municipality where their permanent residence is registered.

QUESTION
I am submitting an application for citizenship and I am from Germany. Do the documents need to be apostilled? Do I need to provide proof of insurance, or is the European Health Insurance Card sufficient? Thank you for your help.
ANSWER:
Hello,
According to the information provided to us by the relevant department of general internal administration, German documents do not require an apostille.
As for the second part of your question, a copy of your health insurance card will suffice.

QUESTION
Hello, I would like to ask about the documents required for the application for Slovak citizenship. I already have the confirmation of my employment and a copy of my employment contract. Now, my employer is asking whether it would be sufficient if they issue one confirmation stating that all my taxes and contributions (health, social, and pension insurance) are paid, or do I need to obtain these confirmations separately from the social insurance company, health insurance company, or tax office?
ANSWER:
Hello,
Thank you for your inquiry.
When applying for Slovak citizenship, in addition to residence and employment documents, it is often required to demonstrate compliance with tax and contribution obligations. It is also necessary to provide confirmation of income tax payment, which is issued by the tax office; confirmation of health insurance payment, which is issued by the health insurance company; confirmation of social insurance payment, which is issued by the social insurance company; and pension savings. Employer confirmations are not sufficient; original documents are required.
