How do I obtain a passport after acquiring Slovak citizenship through my ancestors?

I was granted Slovak citizenship based on my application for citizenship after my great-grandparents. I submitted it at the embassy in the US, where I now live. I speak Slovak at a communicative level. Now the embassy has contacted me to say that I should choose a free date to collect the document and take the oath, and let them know if I want any additional consular services on that date. I want to apply for a Slovak passport, but before I let them know, what is the procedure and what documents do I need? Thank you in advance for your help.

The process of obtaining a Slovak passport in this case involves several steps that precede the actual application for a passport. These are:
- Issuance of a certificate of Slovak citizenship,
- Issuance of a Slovak birth certificate.
Only after obtaining these documents is it possible to apply for a Slovak passport, with the certificate of Slovak citizenship and Slovak birth certificate forming attachments to the application.
The certificate of Slovak citizenship is a document proving that you are a Slovak citizen at the time of its issuance. As part of the application, you must submit:
- a copy of your identity document (bring the original for inspection),
- a certified photocopy of your birth certificate with an apostille and translation,
- copies of your parents’ identity documents (if you have them),
- a completed application form,
- a copy of your naturalization certificate (provided by embassy staff after it has been obtained through consular services).
When applying for a Slovak birth certificate, you must submit:
- a completed Birth Registration form
- the person’s foreign birth certificate with an apostille/superlegalization and an official translation into Slovak
- parents’ identity documents (ID cards, passports) – copies
- identity document – copy
- parents’ birth certificates – copies with translation
- parents’ marriage certificate – copy with translation
- if the parents are not married, a consent form for the determination of paternity (official document issued in the country of birth of the child) must be translated; if this is not issued, we recommend attaching a sworn statement from the parents that they are single.
Once you have both documents, you can apply for a Slovak passport either at the embassy or at the police department in Slovakia.
