A document confirming the purpose of residence, usually a marriage certificate.

A criminal record extract from the applicant’s country of origin and from any countries where the applicant has resided for more than 90 days within the last three consecutive years; generally not older than 90 days.

If the applicant is the owner of an apartment or other real estate in the territory of the Slovak Republic, an extract from the Land Register (a printout from the online register is sufficient). If the property is owned solely by the spouse, the spouse must also submit a statutory declaration confirming that accommodation will be provided to the applicant for the entire duration of residence in the Slovak Republic or for an indefinite period.

If the applicant is renting accommodation, either a lease agreement with notarized signatures of all property owners and the foreign national must be submitted, or a statutory declaration of the property owners confirming the provision of accommodation.
